Just read through most of the HOWTO.

But I can't get a sense of how good of an idea it is and how
much work it takes.  It's unclear, for example, how many packages
will be working under 64b and how smoothly everything works.
For example, from the HOWTO:

"..you can run 32bit applications .. in a way which is mostly
 transparent to the user."

Mostly transparent?

And it's not fully clear how apps need to be run in this chroot,
do I need to chroot everytime I want to run such an app, or is
there some sort of automatic mechanism?

I guess the question is really this:

1) About how much work is it to convert a system to 64b?
   (not much, a day, alot, ongoing, need to recompile everything..?)
2) How smoothly does it work after converting?
   (completely, some hiccups, only problems with a small set of proprietary tools..)

Just trying to get an idea of whether it's really worth it for
a casual user who would like the benefit of 64b for a specific app.
