You might want to try SuperRescue:
http://www.kernel.org/pub/dist/superrescue/v2/

If you want to do something less drastic then that, you could try
running something like:

dd if=/dev/zero of=/dev/<drive to test>, and see if it fails.

I'll bet it is due to the drive failing. 

I would try and run dmesg, and look for drive error messages.
It sounds like you might have to replace the drive.
