One more that wasn't on your list:

* Block all cookies and/or third-party cookies, but let you turn them
on temporarily, for session only or permanently for specific sites.

I'm currently using "CS Lite" in Firefox to control cookies:
they're blocked by default, and CS Lite gives me a menu in the
status bar to enable them.  Anyone know a better option?

Cookies also have the same cross-site problem as Javascript, that I
don't have any way of saying "allow cookies/JS from example.com only
when I'm on example.com, not as a cross-site cookie/script when I'm
on some other site." Once you enable a site, it's enabled even inside
iframes on pages from other sites.  Is there a solution for that?
