I haven't done it myself - but my friend set one up for a slightly 
smaller shop (I think about 10 ppl.)  I'll see if I can get more details 
from him, but here's what I can remember:

basic install was relatively painless
   use the mother boards the vendor recommends.
getting a T1 card up front can be more cost effective then waiting
   Watch slot usage - you can fill up the pci slots pretty easily
   Don't forget existing analog handsets will need a port too.
completion of customing scripting and stuff took about 2-4 weeks.
   They did some fairly extensive custom scripting.  He seemed to
   like the scripting language well enough.
He really likes the handsets they bought (Cisco I believe)
   Not the cheapest, but work well
Disk space seemed to be more important then processor
   (I believe he used a ~2GHz celeron)
