Pac Bowl works for me.  384 Kbps down (1.5 Mbps max) and 128 Kbps up.
I get 1.7 Mbps most of the time.  $65/month with 5 static IPs.  You
can get self-install to reduce the install costs, but you have to
press them for it.  Arrange it with their DSL sales people ("Emerging
Products Group").
