There is also the issue of http authentication on some proxies.

I (ahem) heard it works ok. 
On outside box:
# hts -F localhost:22 8880

On inside box:
# htc --debug 3 -F 2222 -P proxy:port outside_IP:8880 -A proxy_login:proxy_pass

$ ssh -l outside_login -p 2222 localhost

Rumor has it that it's wise to set up more than one port (8880,8881,...)
because on version "hts (httptunnel) 3.3" unexpected tunnel interruptions
may clog the port (so you may log on another one, kill the hts process
and restart it.
