[svlug] Modem Install

I have a PCI PnP fax modem.

On Win95, it is configured as  IRQ 09, IO 0x210.
Win95 says that it is connected to Com5.

On Linux, the number of  "cua" devices I can see is 4.
So, to make this modem work on my linux machine,
what should I do?
