Well, I'm not sure what other's would say, but I have a few suspicions. 
To me, it sounds like the card is obviosuly having to problems talking to 
devices. There can be a few reasons for this. The least possible, we 
hope, is that something is defective. That is a last resort...

If it can't talk to the devices on boot, that's the most obvious sign. I 
would have said it was conflicting IDs (both devices on the same), but 
that would stop it from booting at all (although having SCSI PnP 
enabled might fix that and screw Linux). Another possibility is that the 
cables. They might be defective, not plugged in correctly, or commiting 
cross-talk. Cross-talk occurs when A. The cables are to close, B. The 
cables are in a mess with power calbes, or C, the cables are near some 
other electric feild. This is fixed by terminating the devices, which the 
card attepts to do logicly. I'd recomend checking your setup..

You could also ensure the card is all the way in its pci slot, or perhaps 
try a different slot (unlikely to fix it). In any event, its hardware 
problems, and I doubt its defectivity, but cables, jumpers, or whatever...
